Your 2019 IRS Form 1095-C is now available on myPay.
If you worked for more than one civilian agency, component, or changed pay status (such as retiring or separating from the military) during the tax
year, you may have more than one IRS Form 1095-B and/or 1095-C issued for the same year. Due to differences in payroll processing all of your IRS
Forms may not be available on myPay the same day. Please refer to the Tax Statement Schedule located on the myPay homepage for availability dates.
DFAS is providing your Form 1095-C with information supplied by your healthcare provider per the Affordable Care Act (ACA). The 1095-C will NOT
list your dependents on it. Your health care provider will issue a form 1095-B that will list your dependents and can be used when completing your
taxes.
If you have any questions regarding the information on your IRS Form 1095-C, please contact your customer service representative, installation
representation, or payroll office as appropriate.
If you do not have a Password for accessing myPay, you can obtain one by clicking on the “Forgot or Need a Password?” link on the myPay website at
the web address shown above to receive a temporary Password. If you have any questions concerning myPay, please call our Customer Care Center toll
free at 1-888-DFAS411 (1-888-332-7411), commercial 216-522-5096 or DSN 580-5096, 8:00 a.m. – 5:00 p.m. Eastern Time Monday-Friday (except
holidays).
